When you book your initial appointment with Integrity Family Chiropractic, we encourage you to click the link below to complete your new patient paperwork online prior to your appointment. You are welcome to arrive 10-15 minutes early to complete the forms in our office, if you prefer.
There is no need to wear any special attire. If you’ve had recent imaging, such as X-rays or MRIs, bring the images or reports to your first appointment.

Your First Visit
When you arrive at our office, we’ll greet you with a smile and make you feel right at home. Dr. Burke will take you into a private consultation room and talk with you about your health history, what’s going on with your health currently, and what your goals are for care. He’s a skilled listener, and he’ll probably ask some probing questions to help get to the heart of the problem.
Next, he’ll conduct a thorough examination, which will include assessing your posture, range of motion in specific movements as well as checking your spine for dysfunction. Dr. Burke will explain what he finds and give you his recommendations for care—he’ll let you know what’s causing the problem, what he plans to do to correct it, and how long that will probably take.
If you’re ready to begin, we’ll start your care plan at this visit. This will usually include doing some muscle work first to help relax your body before performing your adjustments.
Paying for Care
We accept cash, check, all major credit cards, and HSAs and FSAs. We also offer a discount for first responders and have different package deals for incoming patients. There is a 3% fee for card payments unless if you are on a wellness plan. Charge is waived for 1x/mo wellness visit.
